Polypharmacy is a crisis your EHR can’t solve.
When patients see multiple specialists, each prescribes independently. Nobody reconciles. That’s a patient safety problem — and it lands back on you.
Fragmented prescribing
The cardiologist, rheumatologist, and endocrinologist each prescribe from their own silo. No single person has visibility into the full list — not even the patient.
Preventable interactions
Drug–drug and drug–disease interactions cause over 125,000 deaths annually in the U.S. Most are preventable with a complete, up-to-date medication list and a coordinator who checks it.
The reconciliation gap
After a hospitalization or specialist visit, medication lists routinely diverge. Patients take discontinued drugs, miss new ones, or double-dose. No one catches it until a crisis.
How the Care Team handles medication management
ElderberryMD’s care coordinators do the legwork — so you get a verified, up-to-date medication list without adding staff time.
Request from every prescriber
The Care Team contacts each specialist and prescriber on the patient’s record to obtain their current medication list. We follow up until we have a complete picture.
Reconcile and flag conflicts
We cross-reference every medication for duplicates, dosage conflicts, and potential interactions. Anything requiring clinical judgment is flagged to you for review — we surface the issue, you make the call.
Deliver the verified list
A clean, reconciled medication list is shared with you and — in plain language — with the patient. Both versions are maintained in the patient record and updated at every prescription change.
Monitor and update continuously
When a specialist adds or changes a prescription, the Care Team captures the change, reconciles again, and notifies your practice. The list is never more than one visit out of date.
Why this works where other approaches fail
Digital tools surface the problem. ElderberryMD’s Care Team resolves it.
Real coordinators, not software
Medication reconciliation requires phone calls, follow-up, and judgment. Our Care Team — real medical assistants and care coordinators — makes those calls and builds the relationships with specialist offices that automation cannot.
Works across all your patient’s providers
We coordinate with every prescriber your patient sees, regardless of their EHR or health system affiliation. The medication list is complete or we keep working.
Included in your practice plan
Medication management is part of ElderberryMD’s core care coordination service — not an add-on. Your eligible patients are covered from day one.
Our role and your clinical judgment
ElderberryMD’s Care Team collects, organizes, and presents the complete medication picture. We flag potential interactions and conflicts for your review — but all clinical decisions remain with you, the treating physician. We do not prescribe, adjust, or discontinue medications.
For medical emergencies, patients are always instructed to call 911. We do not provide emergency or acute clinical care.
Included in the Pro plan
Medication management is part of ElderberryMD’s full-service Pro tier for physician practices.
